Diabetic Retinopathy and Intravitreal Injections

by Nov 21, 2023

Diabetic retinopathy is a progressive eye condition that affects individuals with diabetes. It can lead to vision loss and even blindness if left untreated. Injections of medication into the eye, known as intraocular injections, have proven to be an effective treatment option for diabetic retinopathy. In this blog, we will explore when these injections are needed and how they can help manage this condition.

Understanding Diabetic Retinopathy

Diabetic retinopathy is caused by damage to the blood vessels in the retina, the light-sensitive tissue at the back of the eye. Over time, high blood sugar levels in individuals with diabetes can cause the blood vessels to weaken and leak fluid or bleed. The two main types of diabetic retinopathy are non-proliferative and proliferative.

– Non-Proliferative Diabetic Retinopathy (NPDR): In the early stages, NPDR may not require immediate treatment. The blood vessels in the retina may leak fluid or develop small bulges called microaneurysms. At this stage, lifestyle modifications, regular monitoring, and control of blood sugar levels are typically recommended.

– Proliferative Diabetic Retinopathy (PDR): PDR is a more advanced stage, characterized by the growth of abnormal blood vessels on the surface of the retina. These new vessels are fragile and prone to bleeding, leading to severe vision problems. PDR typically requires more aggressive treatment approaches, including intraocular injections.

Intravitreal Injections for Diabetic Retinopathy

Injections of medication directly into the eye can effectively manage diabetic retinopathy by targeting different aspects of the condition. The two primary types of intraocular injections used are:

  1. Anti-VEGF Medication: Vascular endothelial growth factor (VEGF) is a protein that promotes the growth of abnormal blood vessels in the retina. Anti-VEGF medications work by blocking the action of this protein, inhibiting the formation of new blood vessels and reducing leakage. These injections help prevent further damage to the retina, preserving vision and preventing complications associated with abnormal vessel growth.
  2. Steroid Injections: Steroids are potent anti-inflammatory agents that can reduce swelling within the eye caused by diabetic retinopathy. The injections help alleviate macular edema, which is the accumulation of fluid in the macula, the central part of the retina responsible for sharp, central vision. By reducing edema, steroid injections can improve vision and prevent further vision loss.

Determining the Need for Injections

The decision to intervene with intraocular injections for diabetic retinopathy depends on various factors and requires the expertise of an eye doctor. A comprehensive eye examination, including imaging tests, is essential to assess the stage of diabetic retinopathy and the presence or absence of macular edema. Some of the factors considered include:

Visual Acuity: If diabetic retinopathy is causing vision loss that impacts daily activities, injections may be recommended.

Severity of Non-Proliferative or Proliferative Retinopathy: The stage and progression of diabetic retinopathy play a significant role in determining the need for injections. Proliferative retinopathy often requires more aggressive treatment.

Presence of Macular Edema: When macular edema is present, injections can help reduce swelling and improve vision.

Response to Previous Treatments: If other treatments such as laser therapy or eye medications have failed to achieve desired results, injections may be necessary.
